We are a social services agency providing resources and support to individuals and partner organizations. AT accepts referrals by our community partners. We are goal oriented to positive outcomes.
Vision
Too many seniors are vulnerable and left unsupported. We provide a safety net for your clients by offering resources and assistance to meet their needs by filling in the gaps. Some may need a phone call with a list of resources and education on connecting with those resources while others may need an entirely hands-on approach to success. We provide temporary intensive care management. We are here to help.

Fees
Community Partners
Care Manager and community partner working together to create a care plan identifying present and future support needs. A retainer is required before any work can be undertaken; services, travel, and expenses will first be billed against the retainer before any additional costs are invoiced.
Care Management
Charges based on the actual time spent providing services and includes face-to-face encounters, all phone contacts, correspondence by USPS and email, and certain forms and paperwork. Time is invoiced in 6 minute increments.
Travel Time plus Mileage
When services are performed outside the AT office, there is a charge for travel time to and from the location where services are rendered.
